Teacher/Daycare Request Form

Teachers and day care providers in the Sauk Prairie area may use this form to request books on a particular theme.

Please provide 1 week of advance notice to ensure you request can be filled by the date needed.
Items will be waiting at the Prairie du Sac Library.
Materials will be held for 1 week from the date needed. Items not picked up will be returned.
